Q. How do I change my primary care provider (PCP)?
A. There are two ways you can change your PCP.

  • Go to MyPassportHealthPlan.com and follow the instructions. If you have not registered for MyPassportHealthPlan.com yet, you can do it online today. It’s easy!
  • You can also call Member Services at (800) 578-0603.
