  Provider Center: Self-Enrollment Instructions
  1. At "Provider ID" - enter your Provider ID# or your Group ID#*
  2. At "Password" - enter your federal tax identification number (TIN)
  3. After entering the secure site, follow the prompts to change your password
  4. Enter your password hint and e-mail address

    If you are unable to enter the Provider Center, please contact Provider Services at 1(800)578-0775.
DEFINITIONS/HELPFUL HINTS
*Multiple Office Locations - Be cautious of password usage. If you have two or more offices that need to access the site simultaneously, you will need to carefully coordinate administration of the password. Without this coordination, it is possible to encounter one office canceling out the others' password.
