- Effective 12/1/2004 Passport Health Plan will pay
$22.50 for the Flu Mist (CPT Code 90660) and $5.00 for
the administration of the vaccine (CPT Code 90473).

On December 8, 2003, President George Bush signed into
law the Medicare Prescription Drug, Improvement and
Modernization Act (MMA) of 2003. This landmark legislation
provides seniors and people living with disabilities
with a prescription drug benefit, more choices and better
benefits under Medicare.

- Effective as of January 2nd, 2004--NEW EPSDT Screening
Form
Please use only the NEW EPSDT Screening Form, NOT the
CMS 1500 form when filing all EPSDT screenings. If both
forms are used, your claim will be denied as redundant.
Continue to use the CMS 1500 form for childhood and
adolescent immunizations.
