Letter of Intent - Improved Health Outcomes Program (iHOP)

Letters of Intent

  • Letters of Intent are due by no later than 5:00 PM on Thursday, November 1, 2011. LOI's received after 5:00 PM that day will not be considered for funding.
  • Letters of Intent are to be no longer than 2 pages in length, a 1 page cover sheet and a 1 page narrative section. Please utilize 12 point font and 1 inch margins when preparing the Letter of Intent.

 

Cover Sheet - the cover sheet should be no longer than 1 page and include:

  1. The title of the project
  2. The name of the Principal Investigator
  3. A name and address for correspondence purposes
  4. A phone number
  5. A fax number
  6. An email address
  7. A list of any other investigators
  8. The amount requested

 

Narrative – the narrative should be no longer than 1 page and will NOT be considered for funding if it exceeds 1 page. The narrative page should include the following sections:

  1. Objective(s) of the study and related hypothesis.
  2. Significance of the proposed study (Why should it be done? What are the benefits for patients, organizations or policy development?)
  3. Relevance of the study related to the objectives of the RFP?
  4. Expertise of the Investigator and team members related to this project.
  5. Basic Study Design and Methodology.
  6. Brief Budget Summary.
  7. Feasibility and Potential Pitfalls.
