The Passport Health Plan Behavioral Health Care Coordination program assists providers by incorporating services adaptive to the changing needs of members. Registered nurses and licensed social workers strive to produce active and ongoing participation from members, family, and providers.
Who Is Eligible For The Program?
Passport Advantage members may be considered for this program if they present behavioral health issues or serious physical health conditions that can exacerbate or interfere with behavioral health treatment. A health risk assessment is used during enrollment to identify members who may need assistance. However, provider referrals to our program are also strongly encouraged.
