Special Programs to Keep You Healthy

Your health is very important to us. Pay close attention to the special programs that Passport Advantage offers you. They can help you with your care. Some of the programs are:

• The Chronic Obstructive Pulmonary Disease (COPD) Management Program
  (if you have lung disease).
• The Coronary Artery Disease Management Program
  (if you have heart disease).
• The Diabetes Disease Management Program
  (if you have diabetes).
• The Case Management Program
  (if you have one or more health problems you need help with).
• The Behavioral Health Program
  (for members with emotional, mental, substance and/or drug issues)

These special programs have nurses who can answer your questions. They can help you learn how to take care of your health, get medicines and supplies, and find care from special providers when you need them. Passport Advantage offers you these special programs because we want you to stay as healthy as possible.

• After you enroll, you will need to fill out our Health Risk Assessment Form. The information you give us on this form will help us decide what special programs, if any, are right for you.
