Chronic Obstructive Pulmonary Disease (COPD) Management Program

We are proud to offer a Chronic Obstructive Pulmonary Disease (COPD) Management Program for our members with COPD.

What is COPD?

  • Chronic: means it will not go away.
  • Obstructive: means the airflow out of the lungs is partly blocked.
  • Pulmonary: means it affects the lungs.
  • Disease: means illness or sickness.

What does this program do?

  1. A COPD Disease Management nurse is available to answer your questions. He or she can help you:
    • Understand your COPD.
    • Understand your COPD medicines.
    • Get the supplies and equipment you need.
    • Find care from special providers who work with COPD.
  2. Helpful COPD information may be sent to you several times a year.
  3. Information about your health may be sent to your provider.  Your provider is an important part of caring for your COPD. Please talk to him or her any time you think your COPD is out of control.

If you have any questions about lung disease, call the COPD Disease Management nurse at 1-800-578-0603, press 0 and then press 78215. You can call Monday through Friday, 8:00 a.m. to 4:30 p.m. TTY users please call 1-800-691-5566.
