Heart disease is the #1 cause of death in Kentucky. Adults with heart disease are at risk of having a heart attack or stroke. Our Coronary Artery Disease Management Program is for Passport Advantage members who have heart disease or history of a stroke.
If you have heart disease or have had a stroke, we want to help you get better. If you do not have heart disease, we want to help you avoid it.
What does this program do?
- A Coronary Artery Disease Management nurse is here to answer any questions you have. He or she can help you:
- Understand your heart and stroke disease.
- Understand your medicines.
- Understand how to control your risk factors.
- Find care from a cardiologist (heart provider) or other specialist.
- Helpful heart care information may be sent to you throughout the year.
- A nurse may let your provider know about your coronary artery disease. Your provider is an important part of your care. Please talk to him or her any time you think your heart disease is not under control.
