A formulary is a list of prescription medicines that a health insurance plan covers. The Passport Advantage formulary has many medicines so that you and your provider can choose the right one for you.

Formulary Rules
We have the following rules for using drugs on our formulary.
- We will ask you to use the generic version of a medicine before you use the brand medicine.
- Sometimes, you will only be able to get certain amounts of a medicine. For most medicines, you will get up to a 30-day supply for each prescription. However, for some medicines, you will get a limited supply for each month.
- Sometimes, we will ask you to try using a certain medicine before we will cover another medicine.
- Your provider must ask us before prescribing some medicines for you. This is called “prior authorization”.
If we change our formulary and remove a drug you are taking, we will help you change to a covered drug. Before we change our formulary, we will tell you and your provider 60 days before we make the change. If your provider agrees that you could use a different drug that is on our formulary, your provider will give you a new prescription. If your provider does not agree, you and your provider may ask for an exception. If you have not changed to a new medicine or asked for an exception before our formulary changes, we will let you refill your prescription one time for up to a 30-day supply.

How Passport Advantage created the Formulary
When we created our formulary, we asked our medical staff and network providers for their advice. Our formulary has the medicines that are required by the Medicare program. The Centers for Medicaid & Medicare Services has reviewed and approved the Passport Advantage formulary.
We review our formulary often. When we make a change to our formulary, we want to let you know about it. We will post any changes on this web page 60 days before we make the change.
