University Health Care, Inc. (Passport Health Plan) celebrates 10 years and awards Medicaid Care Improvement Grants

“iHOP (Improved Health Outcomes Program) created as a result of Passport’s successful cost savings and care management programs.”

Grant Summary

Louisville, KY - University Health Care, Inc. dba Passport Health Plan announced today the grant recipients for the organization’s Improved Health Outcomes Program (iHOP).  Grants of $50,000 each were awarded to the following organizations:

  • Family Health Centers and the Center for Health Equity
  • University of Louisville’s School of Nursing
  • Children and Youth Project
  • University of Louisville’s Departments of Family and Geriatric Medicine
  • University of Louisville Hospital Newborn Nursery

Passport Health Plan, now in its 10th year of operating as a Medicaid Health Plan in Jefferson and 15 surrounding counties in Kentucky, has proven extremely successful in improving health outcomes for Medicaid and Kentucky Children’s Health Insurance Plan (KCHIP) members while controlling rising costs for the Commonwealth.

Larry N. Cook, MD, Executive Vice President of Health Affairs for the University of Louisville and Chairman and Chief Executive Officer of University Health Care, Inc (Passport Health Plan) said, “Ten years ago, we started this health plan with the mission to improve the health and quality of life for Medicaid members.  Thanks to the support of our provider and community partners, we have been extremely successful in improving health outcomes for these members.  We’re pleased to be able to extend our efforts by awarding grants for these outstanding programs.”

Mark Pfeifer, MD, Professor and V.V. Cooke Chair of Medicine at the University of Louisville, chaired the selection committee.  “We received 51 applications and it was very difficult to choose the top 5 out of so many outstanding submissions.  Clearly, this response shows community-wide commitment, innovation, and dedication to our most vulnerable citizens.”
