- Take your child’s birth certificate
- If you do not have a copy of this, your caseworker may be able to help you get one.
- Take your child’s social security card
- If you do not have a copy of this, you can visit www.ssa.gov or contact your local social security office to obtain a copy.
- Take proof of child care expenses
- Take copies of receipts or statement from the child’s care providers.
- Take health insurance information if your child has been insured during the last 6 months
- Name of insurance company
- Group number and policy number
- Effective date
- Name of policy holder
- Names of people who are covered
- Take proof of income
- For wages, take copies of pay stubs for the last two months for all people in the household.
- For unearned income such as disability, Social Security, pension, etc., take the most recent award letter.
- Take proof of insurance if your child has been insured in the last 6 months.
What to Expect at DCBS
A caseworker will interview you to see if your children are eligible for KCHIP benefits. He or she will answer your questions, give you information, and may ask for more information or verification. If you are asked to provide more information, you must do this within 10 days or your first appointment.
When your information is complete, you will receive a letter telling you if you have been approved and if you owe a low-cost premium. If you are not approved, you will receive a letter explaining why your child was denied benefits.
