Diabetes Disease Management Program

We want to help all Passport Health Plan members with diabetes stay as healthy as possible, so we created the Diabetes Disease Management Program.

What does this program do?

  • A Diabetes Disease Management nurse is here to answer any questions you have.  He or she can help you:
  • Understand your diabetes.
  • Understand your diabetes medicines (insulin and pills).
  • Get supplies and equipment (test strips, glucometer, and syringes).
  • Find care from special providers, like endocrinologists, podiatrists, and ophthalmologists).
  • You may get helpful diabetes information sent to you in the mail several times a year.

A nurse may let your provider know about your diabetes.  Your provider is an important part of your care.  Please talk to him or her any time you think your diabetes is not under control.

If you have any questions about diabetes, call the Diabetes Disease Management nurse at 1-800-578-0603, press 0 and then press 7074. You can call Monday through Friday, 8:00 a.m. to 4:30 p.m. TTY users please call 1-800-691-5566.
