- Do you or a family member have medical problems?
- Do you need help coordinating your healthcare?
- Do you or your family member have more than one healthcare provider?
If you answered yes to any of these questions, case management
may be helpful to you. Case management is a program for members
who have
one or more health care needs or disabilities. A case manager,
who is a registered nurse or medical social worker, can talk
with you
about your health care needs. He or she can also work with your
healthcare providers to help you get the care you need.
If you think you or your family member(s) would benefit from case
management, please give us a call. You can call us at (502)585-7915
or 1-800-578-0636 ext. 7915. If you are a person with a hearing
impairment, please call our TDD/TTY number at 1-800-691-5566.
