Case Management
If you have special health care needs or a disability, case management
may help you. Passport Health Plan’s case managers are registered
nurses or social workers. They can give you health information and
help you through major health problems. A case manager will work
with you and your provider to help get the care you need.
Examples of members who might need case management are:
- Members with several health problems.
- Members who have cancer.
- Members who are HIV positive or have AIDS.
If you think case management might help you, call 1-800-578-0603
and press 4 at any time, then extension 7915. A case manager will
call you to talk about your needs. Being a part of case management
is your choice.
Asthma
Health Management
Passport Health Plan has a special program for members with asthma.
An asthma manager is here to answer your questions and work with
you and your PCP to help control your asthma. Members with asthma
receive materials in the mail about:
- Things that make asthma worse and how to avoid them.
- Supplies that will help control asthma (peak flow meter and
asthma action plans).
- Asthma medicines.
Asthma Health Management works with providers to let them know
about your asthma benefits. If you have questions about the program,
please call the asthma manager at 1-800-578-0603 and press 4 at
any time, then extension 8307 or 8311.
Diabetes Health Management
Passport Health Plan has a special program for members 18 years
and older with diabetes. The diabetes manager can answer your questions
and work with you and your PCP to help control your diabetes. The
diabetes manager can:
- Answer questions about your diabetes and what you can do to
help control it.
- Help you get supplies you may need.
- Help you access diabetes education classes.
- Remind you about specific tests related to your diabetes.
- Work with your PCP to help meet your individual needs.
The diabetes manager will work with you and your PCP to help keep
your diabetes from controlling your life. If you have any questions
