| You will get two ID cards in the mail. One is a
Passport Health Plan ID card and the other is a Medicaid ID card.
It is important that you keep both ID cards with you at all times.
Your Passport Health Plan ID card will be mailed to you within
10-15 working days of the date you become eligible. You will only
get one Passport Health Plan ID card unless some of the information
on the card changes.
When
you get your ID cards in the mail, check all the information listed
on the cards. You will need to check the information for each family
member. Each family member will receive his or her own Passport
Health Plan ID card. If there is an error on the Passport Health
Plan ID card, call Member Services.
Each member will receive his or her own Medicaid ID card. If there
is an error on the Medicaid ID card, call DCBS or Social Security.

Things to Remember About Your ID Cards
- Take both ID cards to each appointment.
- The ID cards can only be used by the member whose name is on
the card. Do not let anyone else use your card! If you do, you
may be responsible for their costs, or lose your eligibility for
Medicaid.
- Your provider may ask you to show a picture ID. This is to make
sure the right person is using the card.

- Name(s) of eligible member(s).
- Case number.
What’s on the Passport Health Plan ID Card
Some important things on the front of the card:
- Effective date – this is when you can start using your
card.
- Name of eligible member.
- Passport Health Plan ID number.
- Primary Care Provider (PCP).
- PCP’s phone number.
Some important things on the back of the card:
- Passport Health Plan’s Member Services phone number.
- Nurse Advice Line phone number.
