Urgent Care Notice

Passport Health Plan Members

Passport Health Plan now covers after-hours, weekend, and holiday visits to the urgent care centers listed below without referrals from primary care providers (PCP). Times covered without referrals include:

  • Saturdays
  • Sundays
  • National Holidays
  • Mondays through Fridays, after 4:00 p.m

You MUST use the following urgent care centers for the above times to
be covered without a referral:

  • Camelot Immediate Care Center, Louisville, Kentucky
  • Dorsey Lane Immediate Care Center, Louisville, Kentucky
  • Preston Highway Immediate Care Center, Louisville, Kentucky
  • Bardstown Road Immediate Care Center, Louisville, Kentucky
  • Jeffersonville Immediate Care Center, Jeffersonville, Indiana
  • CareFirst, Radcliff, Kentucky

For visits to any urgent care center before 4:00 p.m., Mondays through Fridays, PCP referrals are required.

* An urgent care problem is NOT life threatening but needs immediate attention. If there is time, call your PCP first. If you do go to an urgent care center, call your PCP for follow-up care. Do not go back to the urgent care center for follow-up care.
