Passport Health Plan is a Medicaid health plan for 16 counties
in Kentucky. You became a Passport Health Plan member because you
live in our service area and are eligible for one of the Medicaid
programs we cover.
Passport Health Plan is a health plan that gives you choices –
from choosing your primary care provider (PCP) and pharmacy to participating
in special programs that help you stay healthy. We encourage you
to use the Plan’s many benefits and programs. This Member
Handbook will explain your Passport Health Plan benefits and services.
The Member Handbook gives you information about your benefits including:
Your
first step as a Passport Health Plan member is to choose a PCP.
You can select your PCP from the Primary Care Provider & Direct
Access Services Directory inside this packet. You can choose the
same PCP for each member of your family or you can select a different
PCP for everyone. Your PCP choice depends on whether a PCP is taking
new patients. Call Member Services at 1-800-578-0603 to give them
your PCP choice. They can let you know if a PCP is accepting new
patients. If you’re not sure which PCP to choose, Member Services
can help you. Once you have selected your PCP, be sure to call and
make a routine appointment to get to know him or her. If you are
covered by both Passport Health Plan and Medicare, or you are a
member in out-of-home placement, you do not have to choose a PCP.
However, we strongly encourage every member to have one provider
who manages all his or her health care.
Your Passport Health Plan ID card will be coming in the mail. You
should receive the card within 10-15 working days. It will list
your PCP’s phone number on the front side. You will also receive
a Medicaid ID card.
If you have any questions about your benefits you may call Member
Services. A Member Services representative will be happy to help
you. They are available Monday through Friday, between 8 a.m. and
8 p.m. (EST). The Member Services telephone number is on your Passport
Health Plan ID card and at the bottom of each page of this Member
Handbook.

- Call your PCP for an appointment
- Call Member Services with questions
