KCHIP (Kentucky Children's Health Insurance Program)
The mail in application is a six page document which requires the applicant to provide proof of income, citizenship, identity and proof of daycare expenses. The application can be downloaded from the KCHIP website and either mailed or faxed with appropriate information. The applicant will be given up to 30 days to process DCBS will begin accepting the new application beginning November 1, 2008. Face to Face interviews will be accepted at the local DCBS offices if applicants desire to meet with a caseworker.
