Go to Your Local DCBS Office
You may go to your local Department for Community Based Services (DCBS) office (DCBS offices outside of Jefferson County) OR DCBS offices in Jefferson County) in the county where you live. If you are unable to go yourself, you may send a representative on your behalf. Office hours are from 8:00 a.m. - 4:30 p.m., Monday through Friday. Call ahead of time to see if you need an appointment.
