Asthma Disease Management Program

Passport Health Plan has a special program for members with asthma. An asthma manager is here to answer your questions and work with you and your PCP to help you control your asthma.

Members with asthma get information in the mail about:

• Things that make asthma worse and how to avoid them.
• Supplies that will help control asthma (peak flow meter and asthma action plans).
• Asthma medicines.
